New patient guide
Everything to bring and know before your first visit.
What to bring
- Photo ID and insurance card(s) — including any Medicare Advantage or supplemental card
- Referral or authorization, if your HMO, IPA or workers’ comp claim requires one
- A list of current medications and doses, including supplements
- Prior test results, imaging reports or discs (MRI, CT, EEG) and recent records
- A symptom diary or phone video of episodes, if you have them
- Glasses or hearing aids, and a family member or caregiver if helpful
Patient forms
Intake and consent forms will be provided by the clinic before launch (secure online forms or printable PDFs). Until then, please arrive 15 minutes early to complete paperwork.
On the day of your visit
- Arrive 15 minutes early for check-in.
- Expect your first consultation to take longer than a follow-up.
- If testing is ordered, it may be done the same day or scheduled separately, with preparation instructions.
Bringing a child
A parent or legal guardian must accompany patients under 18. Bring school or therapy reports if they relate to the concern.
